All patients must fill out the all of the
New Patient forms. In addition, all new patients should fill out one of the functional outcome study — the one that is more appropriate to your injury depending on location your body. The
Neck Index is for neck patients, the
Owestry is for back patients, the
DASH is for Upper extremity and elbow and hand, and the
LEFS is for a lower body injury. This is to help us research functional outcomes and provide objective and measurable goals for patients.
